Payment & Refund Policy
Deposit
Scheduling a surgical procedure requires coordinating many different resources to ensure a quality experience for the patient. Upon execution of this Invoice of Surgical Fees (the “Agreement”), Spring Plastic Surgery will incur certain costs and expenses to schedule and prepare for your procedure(s). As such, you are required to pay a one-time NONREFUNDABLE deposit of $1,000.00 (the “Deposit”) concurrently with the execution of this Agreement. You agree and acknowledge that the Deposit is NONREFUNDABLE under any circumstances, including failure to obtain medical clearance for any reason.
You also acknowledge that the Deposit is valid for one (1) year. If your procedure(s) do not occur within this one-year period, additional fees may apply, and prices quoted may be subject to change.
Payment and Terms
The total Amount Due includes pre- and post-operative visits and the listed procedure(s) only. All ancillary services such as blood work, X-rays, mammograms, medical clearance, prescriptions, etc., are your responsibility and are to be obtained at your own expense.
The full Amount Due must be paid prior to the scheduled date of your procedure(s). Please contact your coordinator for the specific payment deadline. Failure to pay the Amount Due by the specified date will result in the cancellation of your procedure(s). You are also responsible for any rescheduling or cancellation fees. If someone other than you makes payments towards your procedure(s), that individual must also sign this Agreement and accept financial responsibility for the payments made, subject to the cancellation policies.
You agree and acknowledge that results of medical procedures are not guaranteed. Your payment is for the services provided, not the outcome of the procedure(s). If you are dissatisfied with the results of your procedure(s) or have concerns regarding payments, you should contact Spring Plastic Surgery at (786) 833-1952. You agree to resolve any payment disputes through mediation before seeking a chargeback from any third party.
If you schedule a revision surgery within one (1) year of your procedure(s), you will be responsible for the costs of the operating room, supplies, anesthesia, surgeon’s fees, and other related expenses. For revisions scheduled more than one (1) year after the procedure(s), you will be responsible for paying in full for any revisions, subject to any price changes.
Cancellation
If you cancel your surgical procedure(s), the following refund policies apply:
- 30 days’ notice:
If you cancel your procedure(s) thirty (30) or more days before the scheduled procedure(s) or at any time if no procedure has been scheduled, you will receive a refund of the Amount Due, less the Deposit. If the Amount Due was not paid in full, the refund will consist of the amount paid, less the Deposit.
- 15-29 days’ notice:
If you cancel your procedure(s) between fifteen (15) and twenty-nine (29) days before the scheduled procedure(s), you will receive a refund of fifty percent (50%) of the Amount Due, less the Deposit. If the preoperative visit was completed, an additional $500.00 Post-Op Cancellation Fee will be deducted from the refund. If the Amount Due was not paid in full, the refund will consist of the amount paid, less the Deposit and the Post-Op Cancellation Fee. The refund may also be reduced by any costs incurred from third-party providers, such as the cost of implants.
- 0-14 days’ notice:
If you cancel your procedure(s) fewer than fifteen (15) days before the scheduled procedure(s), no refund will be provided.
You may choose to reschedule your procedure(s) (a “Rescheduled Procedure”), but a $1,000.00 Rescheduling Fee will be applied in addition to the Amount Due. If you cancel any Rescheduled Procedure, a $1,000.00 cancellation fee will apply to each cancellation.
Refunds and Claims
At Spring Plastic Surgery, we strive to avoid disputes, claims, and chargebacks. We have implemented a 0 Tolerance policy to support our community during uncertain times. If you request a refund or change your mind about a scheduled procedure, please contact our accounting department at legal@springplasticsurgery.com. Refunds and claims are processed within 21 working days.
